Tracy Grafton, LCSW, ACHP-SWExecutive Board Member

Tracy is a clinical social worker with over 20 years experience in end-of-life care. Tracy recently joined the Hackensack Meridian Health Palliative Care Program at Ocean University Medical Center after working at Haven Hospice at JFK Medical Center since 2000. After earning her MSW from Rutgers, The State University of New Jersey, Tracy worked in oncology, child/adolescent/adult bereavement, hospice and then palliative care providing support for patients and families facing life-limiting illness across the lifespan. As an Adjunct Faculty member at both Rutgers University and The College of New Jersey, she has taught on Advance Care Planning, Aging, Oncology, End-of-Life, & Bereavement for all ages.

Tracy is a passionate community educator who is active with the Institute for Healthcare Improvement’s “The Conversation Project”, the New Jersey Chapter of the National Association of Social Workers, and the New Jersey Health Care Quality Institute’s “Conversation of Your Life” task force. In recent years, she has been privileged to present at annual conferences for both the New Jersey Homecare & Hospice Association, NASW and serve as faculty in the NASW/GOCCNJ Advance Care Planning Certification Program.
