The ACP Certificate program builds the skills required for effective discussions and documentation of ACP and satisfies the New Jersey Department of Health (NJDOH) annual requirement for staff training.

What is the GOCCNJ ACP Certificate Program?

This on-demand, virtual, professional development program is designed to increase the competence and confidence in staff member’s ability to facilitate conversations about ACP.

  • Participants study at their own time and pace
  • Includes 8 easy-to access, 10-minute video modules for all staff members
  • Access to a library of patient and family education videos to prepare patients and their decision-makers for care planning meetings
  • Upon completion of the ACP Certification Program, staff members receive a personalized certificate of completion
