Led by the Goals of Care Coalition of New Jersey (GOCCNJ), the Ambassadors Program empowers trusted leaders to educate their communities about palliative care for people living with serious illness and their caregivers.

General Program Information

What is the primary goal of the Ambassador Program?
The program helps older adults, patients, and caregivers understand palliative care, distinguish it from hospice, and learn how to access the new NJ FamilyCare Medicaid Community-Based Palliative Care benefit that became available in April, 2026.
What is my role as an Ambassador?
You will serve as a knowledgeable educator, conducting community events using GOCCNJ’s “Living Well with Serious Illness” presentation to help participants feel comfortable asking questions and making informed care decisions.

Support & Resources

What tools will I receive to help me succeed?

You will receive a boxed Ambassador Kit that includes a name badge, canvas tote bag, pens, and several printed patient education materials.

You’ll also have access to the online Ambassador Hub for easy access to all the tools you need, including customizable flyers, outreach templates, presentation slides, and updates concerning the new Community-Based Palliative Care benefit for NJ FamilyCare members.

Will I have a support team?

Yes! The GOCCNJ team is available to assist you with challenges, CBO outreach, and other needs.

Is there a specific timeline for events?

While you can work at your own pace, a typical cycle begins 8–12 weeks before an event for planning, with reporting tasks completed within 48 hours after the event.

What am I not expected to do?

Ambassadors do not give medical advice, act as case managers, guarantee access to services, or answer clinical questions beyond their training.

Am I paid for my time?

We provide an honorarium to all Ambassadors, up to $500, by filling out a form after the training and events you host.

Commitment & Requirements

What training is required?
A 2.5 hour training is required. Following the training, Ambassadors complete our free Advance Care Planning (ACP) Certificate Program and A Walk In The Woods Program. We also provide ongoing mentoring with local palliative care experts.
Where do Ambassadors hold events?
Events will be in trusted local organizations such as senior centers, libraries, faith-based groups, housing communities, and other community-based organizations (CBOs).
What are the core responsibilities after training?
  • Collaborate with GOCCNJ to identify and connect with local CBOs.
  • Schedule, promote, and deliver “Living Well with Serious Illness” events. We are here to help you and will help you with this!
  • Collect participant surveys and share a reflection about your experience.
  • Support other Ambassadors through participating in occasional online meetings.
How long will I be an Ambassador?

As our first Ambassador cohort, we are planning for you to be involved through the 2026 calendar year, if possible.
