What is Advance Care Planning?

Advance care planning is a process, not an event or a document. It is the process of planning for future medical care in the event that you are unable to make your own decisions. During this time, you’ll discuss, decide and document medical preferences with family and healthcare professionals. You will identify and clarify personal values and goals about health and medical treatment that you may or may not want to receive. You can also determine whom you would like to make health care decisions on your behalf in the event you cannot make decisions for yourself.
